Care Van is a mobile community outreach program that offers free immunizations for uninsured children

ages two months to 18 years old. Your child is eligible if he or she does not have insurance, has

insurance that does not cover certain vaccines (underinsured), has Medicaid, or is American

Indian/Native American or Alaskan Native. Child must have immunization/shot record to receive

services (AISD students’ immunization records can be printed from Skyward Family Access.) Visit the

Wylie Children’s Medical Clinic is run by volunteer doctors and nurses who provide low-cost primary

care for children whose parents cannot afford medical care, uninsured children, children with CHIP, or

with Medicaid. Clinic provides pediatric primary care medical services, wellness checkups, and

immunizations (June and July only). Clinic urges patients to arrive early. Parents must bring a photo ID

and proof of income at time of appointment. Cost of clinic visits will be determined on a sliding scale

based on family size and monthly household income. If currently unemployed, a self-declaration form

can be filled out.

PrimaCare Medical Center provides medical urgent care to Collin County residents. Parents can ask to be

enrolled in the Collin County Health Program (you MUST ASK to be enrolled at time of visit). Parents will

need to fill out the application, provide a valid Texas driver’s license with Collin County address, social

security numbers of parent and of the child (patient), and proof of household income. Qualification is

income based. If approved, the cost will be $20 to see the health care professional for most basic urgent

care needs.

TexCare has partnered with Children’s Health Insurance Program (CHIP) and Medicaid to create a

network of doctors that provide regular checkups, shots, eye exams, glasses, medications, dental care,

mental health care, x-rays, lab testing, specialist care, and hospital care to children ages birth-18.

Children must be enrolled in CHIP or Medicaid to be eligible for services. Applications are available on

the TexCare website and employees are available to assist with applications via telephone or in person.

Students with pre-existing conditions are often eligible and encouraged to apply.

Walmart Care Clinic is a primary care clinic run by nurse practitioners which provides diagnosis and

treatment of chronic and acute illnesses, as well as services such as immunizations, physicals,

prescriptions, and health screenings. Walmart Care Clinic will provide care at a cost of $59-$99 per visit

without insurance. Charges may vary and do not include cost of lab tests, immunizations, or other

ancillary services. Insurance is also accepted.

injuries (broken bones) to uninsured persons. X-rays are available only at the Dallas clinic location.

Watermark Urgent Care is a free clinic which recommends a $10 donation to help cover your visit. No ID,
